Download a PDF copy here from the website or pickup a copy at Boulder Creek Hardware.  Turn in completed forms with payment at Boulder Creek Hardware.   AUTOMOTIVE/MOTORCYCLE ENTRIES must provide a copy of their valid Drivers License and Current Insurance (required by Santa Cruz County Parade Permit).  We’ll see you at the parade!
